Emergency Contact Information
CutePetsbySteph Waiver
This Waiver and Release applies to this current and all future visits to the CutePetsbySteph pet grooming salon, hereinafter ("CPBS") and is effective immediately. This Waiver and Release covers any one and/or multiple pet siblings under one/same household. Ihereby waive and release CPBS, its employees, owners and agents from all liability for any injury or damages that I, or my pet, may suffer as a result of the pet grooming process. By my signature below I acknowledge and agree that the above statement of release of liability applies to each of the following categories below. I have also disclosed any and all known dangers associated with my pet, including behavioral and medical issues that may impact the safety of my pet as well as the safety of the salon's staff.
Terms and conditions of services
Tolerance to Grooming Fee: Prices may change if my pet does not tolerate grooming and requires extra time. An additional fee between $5 and $20 will be applied to my bill.
Emergencies:
I hereby grant permission to CPBS to obtain emergency veterinary treatment for my pet, if my pet is deemed to be suffering from a life-threatening or other emergency. For my pet's benefit, CPBS will use the services of the nearest veterinarian. You will be notified as soon as possible if such an emergency arises. Making certain your pet receives immediate emergency care will be the first priority.
Medical Problems:
I understand pet grooming can expose hidden medical problems or aggravate current problems. Signs of aggravation can occur during or after grooming. If my pet displays signs of severe stress that may be dangerous to my pet's health the grooming process will be terminated and I will be called on for immediate pick up of my pet or the “Emergencies” clause above will be utilized if my pet is deemed to need immediate veterinary attention.
Senior Pet Grooming:
I understand senior pets have an increased risk of injury and/or stress while grooming. CPBS will do everything possible to keep senior pets comfortable and I also understand that CPBS will groom my senior pet with health and cleanliness in mind above aesthetics. Grooming of senior or frail pets is done at the owner's risk, CPBS will not be held responsible for sudden health conditions up to and including death.
Tranquilizers:
CPBS will not tranquilize or sedate a pet for grooming. If you and your vet have decided that your pet would benefit from sedation medication for grooming, CPBS staff must be notified upon drop-off that your pet has been given this medication. Medicating your pet for grooming is done at your own risk and CPBS will not be held responsible for any adverse side effects of the medication up to and including death.
Female Dogs:
I certify that my non-spayed female dog who is brought in for grooming is at least two weeks out of her most recent heat. CPBS does not accept females currently in season. An additional charge of $20 will be applied if my female is discovered to be in heat at the time of her appointment.
Fleas & Ticks:
If my pet is found to be infested with fleas or ticks, CPBS will deem it necessary to use a flea shampoo to rid my pet(s) of the infestation. I also understand that any such treatments are not guaranteed to be 100% effective and an additional fee of $10-$20 may be associated with these special services.
De-Matting:
I am aware that if my pet's coat is matted, it may be necessary to shave or de-matt my pet. These procedures may have unpleasant consequences such as "clipper burn", "brush burn", or nicks and cuts. I understand that the de-matting process takes extra time and will add an extra charge between $5 - $20, to my grooming bill. CPBS will use all precautions during these procedures and will not be held responsible for the aforementioned side effects should they occur.
Sensitive Skin:
Some pets have naturally sensitive skin. These pets are more prone to skin reactions like clipper and brush burn than other pets. While CPBS is careful with all of the pets entrusted to them, young and senior animals and pets with light or white skin are especially prone to skin irritations. If these reactions occur on my pet's skin after grooming, I will notify CPBS as soon as possible so they can make appropriate notes in my pet's file to prevent future problems. Oftentimes minor irritations become aggravated and made worse at home by a pet's own actions; such as scooting on the floor, licking, and scratching. Thus, CPBS will be held blameless for such occurrences.
Injury Notification:
If my pet were to sustain an injury at the salon that does not require immediate medical attention, CPBS will always direct my attention to the injury at the time of pick-up or by phone when I am called for pick-up. CPBS agrees to never conceal any damage to my pet and to provide first aid to my pet on the spot.
Refunds:
I understand that CPBS does not issue/give any refunds of any kind for any service performed at The CutPetsbySteph Grooming Salon. I am aware that I can be offered a complimentary service in exchange within 48 hours or 2 business days of the last service.
I Agree that any dispute arising under this contract or any interpretation required of or the enforcement of rights and duties under this contract shall be subject to final and binding arbitration pursuant to the American Arbitration Association ("AAA''). The award of the arbitrator(s) shall be enforceable according to the applicable provisions of the California Code of Civil Procedure. The arbitrator(s) may award damages and/or permanent injunctive relief, but in no event, shall the arbitrator(s) have the authority to award punitive or exemplary damages. By this Waiver, I assume any risk, and take full responsibility and waive any claims of personal injury, death or damage to personal property associated with such services by CPBS including but not limited to grooming, bathing, creative grooming, temporary or permanent damage to my pet’s hair or unsatisfactory results from the said services being provided to me. This Waiver and Release contains the entire agreement between CPBS and myself, and supersedes any prior written or oral agreements concerning the subject matter of this Waiver and Release. The provisions of this Waiver and Release may be waived, altered, amended or repealed, in whole or in part, only upon the prior written consent of CPBS and Stephanie Varela. The provision of this Waiver and Release will continue in full force and effect even after the termination of the services being provided to me, whether by agreement, by operation of law, or otherwise. I have read, understand and fully agree to the terms of this Waiver and Release. I understand and confirm that by e-signing this Waiver and Release, I have given up considerable future legal rights. I have e-signed this Agreement freely, voluntarily, under no duress or threat of duress, without inducement, promise or guarantee being communicated to me. My e-signature is proof of my intention to execute a complete and unconditional Waiver and Release of all liability to the full extent of the law. I am 18 years of age or older and mentally competent to enter into this Waiver and Release. The client, by e-signing below, agrees to have read, understand, and accept the terms of this contract.
No call / No Show Policy
Sometimes surprises happen! If you have an appointment scheduled with us for your pet’s grooming and need to reschedule, please give us a call as soon as possible so that we can offer the space to a client from our wait list.
If you are unable to give more than 24 hours notice, this is considered a “no show” and is subject to our No call/ No Show policy as follows:
• First time- No penalty.
• Second time- No penalty.
• Third and subsequent occurrences: A deposit equal to half the cost of grooming will be required to schedule future appointments.
Please note: Appointments will be automatically canceled if you are 20 minutes late without prior notice.
